パーキンソン病の理学療法介入:系統的レビューとメタアナリシス


 パーキンソン病に対する理学療法の有効性を、無作為化比較試験を対象として、システマティックレビュー、メタ解析で評価したBMJ 2012の論文の紹介です。

 18のアウトカムの半数で理学療法に利益があることが報告されました。コントロール群に比べ理学療法群で歩行速度、 Berg balance scale 、パーキンソン病統一スケールで臨床的有意な改善があったそうです。

Abstract
Objective

 パーキンソン病患者に対して理学療法の介入ありとなしとを比較して有効性を評価する。

Design 
 無作為化比較試験の系統的レビューとメタアナリシス。

Data sources 
 文献データベース、トライアルレジストリ、ジャーナル、抄録集、会議録、参照リストを、2012年1月の終わりまで検索した。

Review methods 
 パーキンソン病患者の介入なしと理学療法介入群を比較した無作為化比較試験(RCT)を対象とした。 2名のレビューアが独立してそれぞれの試験からのデータを抽出した。標準的なメタアナリシスの方法を、介入なしと比較した理学療法の有効性を評価するために使用した。異質性のテストは、使用される様々な理学療法介入の間で治療効果の違いを評価するために使用した。アウトカム指標は、歩行、functional mobilityとバランス、転倒、 臨床的な機能障害と能力障害の測定、有害事象、コンプライアンス、経済的分析でのアウトカムであった。

Results 
 1827名の参加者の39試験が包含基準を満たした。そのうち29試験で、メタ分析のためのデータを提供した。評価された18のアウトカムのうち9つで、理学療法からの重要な利点が報告されている。臨床的に重要だろうアウトカムは、速度(0.04 m/s, 95% confidence interval 0.02 to 0.06, P<0.001)、Berg balance scale (3.71 points, 2.30 to 5.11, P<0.001), 、および統一されたパーキンソン病のスコアだった評価尺度scores on the unified Parkinson’s disease rating scale(total score −6.15 points, −8.57 to −3.73, P<0.001; activities of daily living subscore −1.36, −2.41 to −0.30, P=0.01; motor subscore −5.01, −6.30 to −3.72, P<0.001).であった。別の理学療法介入の間接的な比較では、治療効果は、統一パーキンソン病評価尺度の運動のサブスコアは別として、どのようなアウトカムでで評価しても、介入によって異なっているというエビデンスが見つからなかった。(その内の1つの試験は、異質性の原因であると判別した)

Conclusions

Physiotherapy intervention in Parkinson’s disease: systematic review and meta-analysis

Abstract
Objective
 To assess the effectiveness of physiotherapy compared with no intervention in patients with Parkinson’s disease.

Design
 Systematic review and meta-analysis of randomised controlled trials.

Data sources
 Literature databases, trial registries, journals, abstract books, and conference proceedings, and reference lists, searched up to the end of January 2012.

Review methods
 Randomised controlled trials comparing physiotherapy with no intervention in patients with Parkinson’s disease were eligible. Two authors independently abstracted data from each trial. Standard meta-analysis methods were used to assess the effectiveness of physiotherapy compared with no intervention. Tests for heterogeneity were used to assess for differences in treatment effect across different physiotherapy interventions used. Outcome measures were gait, functional mobility and balance, falls, clinician rated impairment and disability measures, patient rated quality of life, adverse events, compliance, and economic analysis outcomes.

Results
 39 trials of 1827 participants met the inclusion criteria, of which 29 trials provided data for the meta-analyses. Significant benefit from physiotherapy was reported for nine of 18 outcomes assessed. Outcomes which may be clinically significant were speed (0.04 m/s, 95% confidence interval 0.02 to 0.06, P<0.001), Berg balance scale (3.71 points, 2.30 to 5.11, P<0.001), and scores on the unified Parkinson’s disease rating scale (total score −6.15 points, −8.57 to −3.73, P<0.001; activities of daily living subscore −1.36, −2.41 to −0.30, P=0.01; motor subscore −5.01, −6.30 to −3.72, P<0.001). Indirect comparisons of the different physiotherapy interventions found no evidence that the treatment effect differed across the interventions for any outcomes assessed, apart from motor subscores on the unified Parkinson’s disease rating scale (in which one trial was found to be the cause of the heterogeneity).

Conclusions
 Physiotherapy has short term benefits in Parkinson’s disease. A wide range of physiotherapy techniques are currently used to treat Parkinson’s disease, with little difference in treatment effects. Large, well designed, randomised controlled trials with improved methodology and reporting are needed to assess the efficacy and cost effectiveness of physiotherapy for treating Parkinson’s disease in the longer term.

背景:
 理学療法と転倒予防教育は特発性パーキンソン病を持つ人々に転倒や障害を軽減すると言われているが、これはまだ大規模なRCTで調査されていない。この研究では、(ⅰ)転倒予防教育を組み合わせた運動戦略研修、(ⅱ)転倒予防教育と組み合わせて漸増抵抗筋力トレーニング、(ⅲ)一般的なライフスキル、社会的プログラム(対照群)の転倒、モビリティ、QOLにおける効果を調査する

メソッド/デザイン:
 自宅に住んでいる特発性パーキンソン病患者を集め、ランダムに3つのグループのいずれかに割り当てる。それぞれの対象者は3-4人のグループで外来で治療を受ける。各グループは、8週連続で週1回2時間会うようにスケジュールされる。すべての参加者はまた、8週間の介入期間に週ごとの構造化された2時間の自宅練習プログラムがある。評価は治療前に、8週間の治療プログラムの後、3ヶ月と12ヶ月の介入後に行われる。参加者のそれぞれは転倒の数の記録を外来治療のあと12ヶ月続ける。 Prevention of Falls Network Europe group,の勧告に沿って、3つの転倒の変数が、主なアウトカム指標として使用される。転倒の数、複数の転倒と転倒率。転倒を定量化することに加えて、我々は副次アウトカムとして、モビリティ、活動制限と生活の質を測定するものとする。

議論:
 この研究で、転倒予防教育や転倒予防教育を組み合わせた漸増抵抗筋力トレーニングを組み合わせた外来患者の動きの戦略トレーニングが、自宅に住んでいるパーキンソン患者の転倒数を削減し、モビリティと生活の質を向上させるために有効であるかどうかが明らかになる可能性がある。

パーキンソン患者の歩行に対する新しい挑戦:シングルおよびデュアルタスク環境での同時音楽の効果

Intervention
被験者は4つの異なるテスト条件で遮るもののない通路に沿って自己選択したペースで歩いた。  実験条件は、音楽の伴奏の有無(無音楽/音楽)と二次認知タスク(シングル/デュアル)の存在によって分けた。 シングルおよびデュアルタスク環境は、無作為化された。実験は、音楽の存在によってブロックされ、対象者間で平衡化した。音楽は、対象者が自分で選んだ。 認知タスクは、数の減算(3の)から成る。対象者は音楽に対応するように指示されておらず、彼らはタスクの優先順位に関して指示も受けなかった

Main Outcome Measures

Results

太極拳とパーキンソン病患者の姿勢の安定性

パーキンソン病患者に対する太極拳の効果を検証した論文の紹介です。レジスタンストレーニング群とストレッチ群とに分けて無作為化比較試験を実施しています。


結果としては、太極拳群はその他の両群よりも優れた結果となり、軽度から中等度のパーキンソン病患者のバランス障害を軽減するようであり、加えて、運動機能を改善し、転倒回数を削減する。とのことでした。

BACKGROUND
パーキンソン病患者は大幅にバランスを損なわれており、機能的能力の減少と転倒のリスクの増加につながる、。練習は、日常的に医療提供者によって奨励されていますが、ほとんどのプログラムが効果的であることが証明されていない。

METHODS

太極拳群は、最大重心移動のおいて、レジスタントトレーニング群とストレッチ群よりも終始、優れていた (between-group difference in the change from baseline, 5.55 percentage points; 95% confidence interval [CI], 1.12 to 9.97; and 11.98 percentage points; 95% CI, 7.21 to 16.74, respectively) そして、運動の正確さにおいてもレジスタントトレーニング群とストレッチ群よりも終始、優れていた (10.45 percentage points; 95% CI, 3.89 to 17.00; and 11.38 percentage points; 95% CI, 5.50 to 17.27, respectively)。太極拳群はまた、ストレッチ群よりも二次アウトカムのすべてでの高いパフォーマンスがあった。レジスタンストレーニング群とではストライドの長さとファンクショナルリーチで上回った。太極拳群は、ストレッチ群と比べて転倒、転落事故を下げた。しかしレジスタンストレーニング群とでは有意な差がなかった。太極拳トレーニングの効果は、介入後3ヶ月の間維持された。重大な不利益事象は観察されなかった。

CONCLUSIONS

パーキンソン病患者の運動行動に関連する要因

パーキンソン病患者の運動行動に関する論文の紹介です。

結論としてはパーキンソン患者では、機能障害よりも自己効力感が定期的に運動するかどうかに関連しているとのこと。パーキンソン患者に対する介入プログラムを設計する際には自己効力感を増やすための戦略を含めるべき。

Abstract

Background

 パーキンソン病(PD)患者の障害を軽減する目的での運動の利点がより明らかになっている。しかし、最適な利益は、定期的で持続的な参加を必要とする。PD患者が、定期的な運動に従事することに関連付けられる要因は科学的な精査を少し受けている。

Objective
本研究の目的は、ガイドフレームワークとして国際生活機能分類(International Classification of Functioning, Disability and Health ;ICF) を用いて、PD患者の運動行動に関連する要因を探ることであった。

Design 
横断的研究

Methods
 本研究では、参加者は4つの施設からのPD患者260例を対象とした。参加者は(Stages of Readiness to Exercise Questionnaire)へ反応に基づいて 「exercisers 」や 「nonexercisers」として指定した。運動の状態は、(Physical Activity Scale for the Elderly and an activity monitor)を使用して検証した。潜在的に運動行動に関連付けられている要因は、身体​​構造や心身機能、活動、参加、環境因子、および個人的因子の程度が含まれている。それらの相対的な貢献は、ロジスティック回帰分析を用いて分析し、オッズ比で定量化した。

Results
 164の参加者(63%)を exercisers とした。自己効力感が高い参加者は(調整オッズ比= 2.34、95%信頼区間= 1.30から4.23)自己効力感が低い参加者と比べて定期的な運動に従事する割合が2倍以上高かった。大学教育を受け、高齢の参加者も運動するする可能性が高かった。障害、活動制限、参加制限の影響を無効化することは運動行動に関連しなかった。

Limitations
 研究の横断的な性質は、因果関係の推論をする能力を制限しました。

Conclusions
 機能障害ではなく、むしろ自己効力感が、PD患者の 歩行可能な、 地域社会に住む人々が定期的に運動するかどうかに関連しているようである。この研究の結果は、理学療法士は、PD患者に対する患者の介入プログラムを設計する際に、運動の自己効力感を増やすための戦略を含めるべきであることを示唆している。





※追記:自己効力感は運動自己効力感スケール(Exercise Self-efficacy Scale;ESS)を評価として使っているようです。質問では、運動を行うことの妨げとなりうる障壁を提示し、障壁を乗り越える自信の度合いを回答してもらう形となっている。例えば(1)疲れている、(2)天候が悪い、(3)家族や友人の助けがない、(4)他にやることがある、などといった項目に回答してもらう。回答結果は、各項目100点満点で、高スコアほど自己効力感が高いことを示す。なお、ESSの総スコアは、各項目の平均値として算出されるようです
